University Wellness is offering employees an opportunity to participate in the Fall 2023 Wellness Incentive Program.
“As a part of the incentive, we are inviting all employees to take advantage of this program,” said Vicki Riddick, director of University Wellness Services. “The Health Assessment is a self-reported survey of approximately 40 questions regarding overall well-being. The survey takes less than 20 minutes. Upon completion, participants will receive an automatic personal wellness report and total wellness score.”
The health assessment can be used as a tool for identifying areas of improvement and to gain a better understanding of how habits affect health. The health assessment is confidential, and only aggregated data will be used for tracking purposes.
This is a voluntary program and participation is up to each individual employee.
Employees currently enrolled in Medical Mutual for 2023:
• Use My Health Plan login to access the Wellness Portal and complete the heatlh assessment between Sunday, Oct. 1, through Tuesday, Oct. 31.
For employees not enrolled in Medical Mutual and would like to complete the health assessment:
• To have access to the Health Assessment on the MMO’s platform, you will need to establish a non-member account; however, you must complete this online form to participate.
